  1. Customers intending to order prescription or DSCSA-regulated products must submit the following documentation for verification:

    1. A completed Customer Rx Ordering Application
    2. A valid Global Location Number (GLN)
    3. A current and valid medical license number listing an address that accurately matches the designated shipping address of your establishment.

  2. To ensure secure handling of sensitive information, all documentation should be submitted via mail or fax. Submission via unencrypted email is not recommended due to potential data security risks.

  3. Data Security Notice:

  1. Please allow up to three (3) business days for the verification and processing of your application. Processing times may vary depending on the completeness and accuracy of the submitted information.
Shipping Restrictions:

  1. Please note that our Rx and DSCSA-regulated products are currently only eligible for shipment within the states of Texas and North Carolina.
